Click on the link below to load/view the fillable Youth Participation Form. Complete the form online within your browser. When the form is finished, if you are able to download the completed form to your computer, do so.  If there is no download option available, select print and then "Save to pdf." This should download/save the completed form to your computer.  Go to the folder that is designated on your computer system as the Downloads Folder and locate the form.  Open your eMail application and attach that completed pdf form to an eMail addressed to youth@stmattlutheran.org with a Subject Line: "Youth Participation Form."  Include whatever additional comments you may have in your eMail message.
